“Much of HF care after discharge is siloed (e.g., hospital discharge, transitional care services, and HF clinic) and we believe engagement with patients and caregivers to place them at the center of care is essential. The post on the global challenge and opportunity in the heart failure community really highlights how uneven healthcare systems are across countries and how much room there is for better coordination and early care. It reminded me of a biology class discussion where we compared how different health systems handle chronic illness. I once used NEBOSH OBE exam help online while trying to manage revision stress, which helped me organize complex topics better. It makes me think that global health problems always need shared learning and steady improvement.
